Preparing a Risk Assessment

Select the sample “Case 1” in the clinical notes.

To start preparing a risk assessment, first select the Mental Health Risks summary type and click summarise.
We recommend working through the individual category tags to understand the risk profile.

Select the “Vulnerability” tag. You will see that there are a number of statements suggesting vulnerability. The first clearly relates to homelessness. The second appears to relate to vulnerability but on inspection is also about risk to others. Note both in your risk assessment document. TwynSyght will draw attention statements that are related, it is the role of the professional to determine if the information provided is actually relevant.

You can continue scanning through the categories and statements in this manner. Quite often you may be interested in the risks over a particular period of time.

Visualise the current summary. You will see that the risk recordings peak between September 2010 and November 2010. To filter this, click “close” then click the options button, and select the appropriate date range.
